Raymond has made great progress starting Saturday afternoon, once his ventilator mode was changed Saturday. We are weaning everything from sedation and pain medicine, to nitric and ventilator settings and Raymond has tolerated it well. Helen and I along with Raymond’s nurses are lobbying for fewer changes tonight to make sure we have time to make sure each change is tolerated well.

Since Saturday Raymond has peed off an extra two liters of fluid that came from his edematous body. This is an answer to a big prayer request from Saturday. His body is starting to look normal size again. His legs and stomach area is still big and his arms are normal size and his face has shrink a lot in the last two days. Raymond abdomen is still large and he needs to poop, he had two big ones two days ago but since has not and his feeds have been restarted and are almost back to full amount.

Raymond has a surgery appointment to get a tracheotomy on Thursday. Today the team came in to examine Raymond and consult on his readiness and need. Raymond needs to be off nitric and his inspiration pressure around 16 or lower which is where we are already. If he continues to need less and less ventilator support the need for a tracheotomy will decrease. But everyone is expecting him to get one and that it will greatly help him heal. Once he has a trach he can come most of his sedation and do more movement, he has a lot of physical therapy to do.

Many of his sedation medicines are getting weaned. This has helped his heart rate, which in turn helps him maintain better blood pressures. He is also moving a lot more and twice today he open his eyes for a few minutes. We hold his hands or give him a syringe to hold and let him move his arms around. The main thing is to not let him pull on any of his IVs or monitor leads. Sometimes he shakes like he is withdrawing. His temperature rose today to above normal for the first time in weeks, to 100.4 at which time he got Tylenol. I guess we always need something to worry about, although we can blame it on withdrawing from morphine and sedation medicine.

Raymond had his second negative PCR H1N1 test come back. I though he might get declared free from H1N1 today but they follow protocol and requiring 3 negatives each 1 week apart. If next Monday's is negative we will met that requirement and he will be declared negative. At that time we will no longer have to wear a mask at all times and we may get to get rid of our gown and gloves.

Enough for today I’ll try to talk more about the trach and the long term care needs that his doctors think that he will need.

Praise for this request it is nearly solved - Pray Raymond’s body overcomes this septic inflammatory process and heals and his veins can absorb all the extra fluid that is his tissue throughout his body.

Thank-you Lord for another week with Raymond we cherish each and everyday with him and are looking forward to reunited him with his sisters at home.

Update for Thursday night, Friday and Saturday

Sorry for no update yesterday. I had to work on both of my daughters computers to get them back up and running. Without theirs working I had to leave mine at home.

The last update I did was for Wednesday evening and Thursday. We indicated things were going relatively good, where the relative is a comparison to the prior days since they were not good days. Well Thursday night he started his antics again with unstable blood pressure and oxygen levels and that continued into Friday where he stayed on the edge of his margin close to where intervention is needed and caused us to stay late in the evening and early hours of the morning. He started out the same way this morning scaring us right off the bat. He was unable to keep his oxygen levels up and over the past couple of days instead of weaning the ventilator he continued to need more support. We were on 80% oxygen and had changed the ventilator settings to allow more inspiration time and this did not seem to be enough. Finally the respiratory therapist changed the ventilator mode from APRV to pressure support and he immediately reacted positively. Within 30 minutes his oxygen levels were great and he was down to 55% oxygen on the ventilator. The reason we switched earlier in the week is he was not able to maintain adequate oxygen and C02 levels and the APRV allowed better

During rounds the last couple of days we had been having serious discussions on some sort of intervention because Raymond’s right lung is not getting better. Wednesday and Thursday the talk was about doing another bronchial scope of his lung airway. See if there are any large plugs of thick mucus that can be extracted even though everyone is aware that we are not suctioning much out his lungs. They feel like we need to get more information on the right lung because it is not healing. They are basically saying that he has unilateral lung function meaning only one functioning lung.

The other talk is that he is septic acting meaning he is acting like he has an infection or inflammatory response in his body affecting his veins and metabolic processes. When you are already in critical condition it does not take much to throw you whole system out of whack. Part of why he is getting so many fluid infusions is his veins have been leaking more the last couple of days because of this inflammatory process. Everyone has been concerned with Raymond progress now. That is why he was started on a vancomycin and zosyn on Tuesday morning when this all started. Even though his blood cultures have been negative.

Several times this week a tracheotomy was brought up for a possible future procedure to eventually get Raymond out of the ICU. Many doctors believe it is the only way Raymond can leave the ICU in a timely manner. One quote from a doctor was “does anyone see Raymond getting extubated any time in the near future”. With a tracheotomy all the upper airway pressure from the palate and upper airway is eliminated and the tube is only about 6 inches long. A ventilator can be attached to it and the pressure settings could be lower. Raymond could come off all his sedation and eventually leave the ICU and move closer toward the ultimate goal of home.

Raymond had a great rest of the day today after we got him tweaked around noon. Putting him back to the pressure support ventilation that more mimics the way we breathe by giving a steady pressure followed by inspiration pressure to start a breath and expiration pressure to aid in C02 exhausting. On APRV he got a constant pressure to keep his lungs expanded followed by a small time of no pressure to exhaust C02 but Raymond had to initiate all of his breaths. This required him to burn more calories because of the additional work. We all think this may have fatigued him especially since his feeds has been stopped for 3 – 4 days because his stomach was distended,

Yesterday Raymond finally stooled a couple of big ones. It started immediately after the resident doctor checked to see if he was impacted. This was a big relief for all, his stomach was much distended. We started back trickle feeds yesterday. At this evenings rounds I asked if we could start increasing his feeds they agreed to start going up periodically as he tolerates and continues to stool.

Another change that helped him was lowering his presidex (sp?) starting yesterday. It has the adverse effect of suppressing the hearts rate. After today's wean it had the desired effect of raising his heart rate giving his blood pressure more stability.

This was a very tough week, Raymond made no progress and actually slid back a little. I think this afternoon and evening stability is the start to a up sustained up cycle.

good day! mom and I are doing great, she comes home today. Thank God for watching over us and little Ray!!
I do need prayer for a mom of one of my daughters friend, she caught some infection while in Brazil and is back in the states but had two strokes and a heart attack yesterday and is not doing well at all. Please pray for the family to have peace and to be comforted should she pass on, they are not giving much hope but we know there is always hope with Jesus! I do not know if they are Christians, asking God to show mercy and give the lady a chance to find Jesus and for the family to see God at work. Love you all.

Sending Angels and Prayers to all those standing n the Need of Prayers with prayers and Supplication Letting our request be known unto God he does answers Prayers. so what every the,Burdens,Illiness, Situation, Problem, Issue may be Give it God and Watch God. If you gave it to him and your still worried you haven't let Go Let Go and let God. God doesn't need your help it's you that need him to help.. Be blessed and let God work his wonders in your Life.
